This article explains to EOR employees how they can use the anonymous reporting feature to report actual or suspected incidents. All reports are treated confidentially, and the reporter's identity and case details will never be disclosed.
Submitting a Report
✅ Step 1 - Go to the Anonymous Reporting tab
Employees can access the tab via the More icon on their homepage.
✅ Step 2 - Click New Report
✅ Step 3 - Fill out the required information
By selecting the option to submit a new report, employees will be asked to submit the following:
- Category - they can choose from a dropdown list
- Date of the incident - if applicable
- Who was involved - the name and job title of those involved
- Witness and relationship to them
- Incident subject
- Description
✅ Step 4 - Acknowledge and submit
After submitting the report, employees will receive a case ID and a six-digit passcode. With that passcode, they’ll be able to follow up on the case.
