Enrolling and managing employee account through Slavic401k
How to enroll
To enroll, head to the Slavic401k website, click on the ‘Enroll’ button at the top of the screen and follow the prompts.
The employee will need to provide their name, date of birth, social security number and employment information before they are able to select their contributions.
How to sign up for online account access
Once the employee is enrolled, they will need to register for online access to manage their account. They can do this by clicking the ‘Login’ button at the top of the Slavic401k website’s homepage and following the prompts to set up their login credentials.
Once complete, the PEO employee can access their account through the Slavic401k website and mobile app.
How to reset the password
To reset the account password, click on the ‘Forgot Password’ link on Slavic401k’s online account login screen to reset the password. Employees can also recover their username if they have forgotten it.
If the employee becomes locked out of their account, please call Slavic401k at (800) 356-3009.
How to see the plan information
Once logged into the online account, click on ‘MyPortal’ and then select ‘Forms’. On the ‘Forms’ page, view the Summary Plan Description document, which contains details of the plan including fee disclosures, employer match information, and more.
Hardship distribution and loan information
Hardship distribution eligibility requirements
In order to qualify for a hardship distribution, the IRS requires that PEO employees meet one of the following conditions:
- Unpaid medical expenses within the past 12 months
- Purchase of a primary residence
- Tuition expenses
- Eviction or foreclosure
- Funeral expenses
- Expenses incurred from a FEMA declared disaster
- Expenses related to damage to the employee's principal residence
How to apply for a hardship distribution
If any of the above conditions apply to the employee, they are advised to follow these instructions to apply:
- Log in to their Slavic401k account
- Click on My Portal
- Choose Hardship Request and follow the prompts.
Please be aware that hardship requests take 10 - 15 business days for processing and an additional 1 - 3 business days to receive the funds electronically, or 7 - 10 business days to receive the funds via standard mail. Overnight mailing is available for an additional fee and is subject to a one-day delay if the distribution is a large amount.

How to apply for a loan through 401(k)
In order to be eligible to take a loan, the employee must be currently employed by the company with which they have their 401(k) account with. If the employee meets the above criteria then these are the steps to apply for a loan:
- Log in to the employee account
- Click on My Portal
- Select Loan Request and follow the prompts.
Please be aware that loan requests take 10 - 15 business days for processing and an additional 1 - 3 business days to receive the funds electronically, or 7 - 10 business days to receive the funds via standard mail. Overnight mailing is available for an additional fee and is subject to a one-day delay if the distribution is a large amount.
How to find the 1099-R Form
Once logged into the online account, click on ‘MyPortal’ and then select ‘Forms’. Employees can download the 1099-R form this page. 1099-R forms will only be available if the employee took a distribution or loan from their 401(k) during the tax year.
